Why does Indiana always rank low in national health care?

According to a 2020 study conducted by Nir Menachemi and Paul Halverson for Indiana University (IU) Indianapolis’ Richard M. Fairbanks School of Public Health, Indiana ranked bottom 10 nationally for health care categories such as vaccinations, mortality rate (infant included), obesity rate and smoking rate.
